During the 1970s, Tengah Island served as a transit campsite for Vietnamese refugees, a period of historical significance for the island. However, the campsite was closed in the early 1980s.

In 1985, Tengah Island was among the few islands nominated by the Malaysian government as a Marine Park, reflecting its ecological importance.

Tengah Island is situated 16 km from Besar Island and can be reached via a 50-minute boat ride. It is home to a resort called Batu Batu, which offers visitors a variety of activities such as snorkeling, scuba diving, and jungle trekking.

Batu Batu resort also runs a conservation program called Turtle Watch Camp, which is aimed at protecting and preserving sea turtles. Marine biologists associated with Turtle Watch Camp have built a hatchery to safeguard sea turtle eggs until they hatch, playing a crucial role in the conservation of the endangered sea turtle population.

The island has also gained popularity as a filming location, being the first site used for the TV show Expedition Robinson in 1997. With its pristine natural beauty and unique cultural significance, Tengah Island is a hidden gem that is worth exploring.
