Chiasmocleis bicegoi
The Cecilia humming frog (Chiasmocleis bicegoi) is a frog that lives in Brazil. Scientists have only seen it in one place.[1][2]

Scientists used to think this was the same frog as Chiasmocleis albopunctata, but they decided in 2018 that it is not.[1]
